Motorcyclist dies after box truck driver turns in Woodside
A 57-year-old box truck driver was turning from Roosevelt Avenue onto 64th Street in Woodside when motorcyclist Jorge Palaquibay hit the truck and died at the scene, police said in press accounts.

The rider hit the truck and was thrown from his Kawasaki; EMS pronounced him dead at the scene. The Daily News reported that video showed the driver moving left before cutting right, while QNS later reported that police said the rider tried to overtake the truck on the passenger side. Reports also differed on Palaquibay's age, with the Daily News reporting 25 and QNS later reporting 31. The turn at Roosevelt Avenue and 64th Street
Police said the rider and the 57-year-old box truck driver were both traveling west on Roosevelt Avenue around 7:45 a.m. when the driver made a right turn onto 64th Street and the rider hit the truck.
The Daily News reported that video showed the driver moving left toward the middle of Roosevelt Avenue before cutting right for the turn. QNS later reported that police said the driver had pulled away from the curb after a delivery before turning northbound onto 64th Street.
A Woodside crash scene by 64-02 Roosevelt Avenue
QNS reported that officers from the 108th Precinct responded to a 911 call for a collision in front of 64-02 Roosevelt Avenue and found the rider on the roadway unconscious and unresponsive.
The impact left the rider at the northwest corner of the intersection, and QNS reported that his motorcycle ended on the sidewalk near Roosevelt Avenue and 64th Street.
Jorge Palaquibay was identified after family notification
QNS reported that the NYPD identified the rider on July 4 as Jorge Alcibiades Palaquibay-Lema of Elmhurst. The Daily News later identified him as Jorge Palaquibay and reported that he was a construction worker, an immigrant from Ecuador, and a father of three young children.
Reports disagreed on his age: the Daily News reported that he was 25, while a later QNS account reported that he was 31.
Investigators examined how the rider and driver approached the turn
The Daily News reported that police were investigating whether the rider was traveling between lanes. QNS later reported that police said the rider tried to overtake the truck on the passenger side, in the driver's blind side, before the impact.
The driver stayed, and police reported no arrests
Police said the driver remained at the scene. QNS reported that the NYPD Highway District's Collision Investigation Squad was investigating, and both QNS and the Daily News reported that no arrests had been made.
